Y2CONNECT 

Y2CONNECT is a resource guide for youth in Baltimore. is an initiative under Project Connect Baltimore that is dedicated to improving the health of adolescents and young adults in Baltimore City.

The site aims to connect Baltimore youth to clinical and community-based health and non-health programs and resources to assist them in becoming successful adults. It is designed for easy and direct access by youth‐serving professionals, parents/guardians, and other caring adults, and youth. 

PrEP is for Youth

PrEP is for YOUth is a program at the Center for Adolescent and Young Adult Health at Johns Hopkins Harriet Lane. Their primary goal is to help young people stay on top of their sexual health.

The PrEP is for YOUth team works with and cares for all communities of young people with a focus on young LGBTQ people of color. This program provides sexual health care that is focused on the needs of adolescents and young adults ages 15-25, regardless of insurance. They stay connected with patients through phone, email, text, and social media. The clinic offers intensive support from peer navigators, free HIV/STI testing, text reminders and wellness check-ins, and free condoms. All services are confidential.

Chase Brexton Health Care

Chase Brexton has a mission of providing care that centers and uplifts individual identies. Their program, The Center for LGBTQ Health Equity focuses on LGBTQ focused health care services, advocacy, trainings/education and publications to further understand LGBTQ health.

Center for Transgender Health

  • The mission of the Johns Hopkins Center for Transgender Health (CTH) is to reduce healthcare disparities and improve the overall health of the transgender community through world-class clinical care, medical education, and research. Embracing diversity and inclusion, the CTH provides affirming, objective, person-centered care to improve health and enhance wellness; educates interdisciplinary healthcare professionals to provide culturally competent, evidence-based care; informs the public on transgender health issues; and advances medical knowledge by conducting biomedical research.

The PRIDE Center of Maryland

  • The goal of The PRIDE Center of Maryland is to be a catalyst for uniting and empowering sexual and gender minorities in Baltimore and Maryland and to advocate for a better quality of life for the entire community. The PRIDE Center runs multiple programs specifically to support trans individuals.

TransMaryland

  • TransMaryland is a trans-led multi-cultural organization focused on supporting Maryland’s trans community. Their services include name change petitions, virtual T4T support groups, and linkage to gender affirming care.

Baltimore Transgender Alliance

PFLAG: Parents, Families, and Friends of Lesbian and Gays

  • 400 chapters nationwide, in nearly every state

  • Provides information on “coming out” for families, friends, and allies. Has resources, terminology, and advice for families

Family Acceptance Project

  • Family support module and research-based resources to help diverse families including conservative families support their SGM children in the context of their values and beliefs

  • Gives youth safe opportunities to connect with each other in spaces moderated by trusted adults

Stand with Trans

  • Organizes events and support groups in Michigan. Provides an online database of resources for trans teens, including Ally Moms, a group of parents teens can contact if their own are unsupportive

Trans Youth Equality Foundation

  • Provides online resources and hosts youth camp and support groups in southern Maine
